Organised by the <strong>Internal Medicine Department<\/strong> at <strong>Hospital Universitari de Bellvitge<\/strong>, the event focused on the need to share experiences, identify common challenges and move towards more coordinated, patient-centred care.<\/p><p><a href=\"https:\/\/hestiahealth.es\/en\/centres\/hestia-health-duran-i-reynals\/\"><strong>Hestia Health Duran i Reynals<\/strong><\/a> was represented by <strong>Dr Jos\u00e9 Manuel N\u00fa\u00f1ez, Medical and Clinical Care Director,<\/strong> who took part in the panel on <em>coordination between levels of care<\/em>. Alongside representatives from <strong>GAPiC Delta<\/strong> and the hospital itself, the panel explored different perspectives on improving connections between primary, hospital and integrated health and social care.<\/p><p>In his contribution, Dr N\u00fa\u00f1ez explained the intermediate care model used at the centre, highlighting its role as a bridge between the acute hospital and the community. He also emphasised the importance of effective coordination in responding promptly to complex clinical situations<em>.<\/em><\/p><p>The conference provided a space for open dialogue between professionals from different disciplines, highlighting the importance of working together to provide care that better meets people&#8217;s real needs.
