The third Update Conference on Managing Patients with Multiple Conditions brought together professionals from different care settings to reflect on how to improve care for people with complex chronic conditions. Organised by the Internal Medicine Department at Hospital Universitari de Bellvitge, the event focused on the need to share experiences, identify common challenges and move towards more coordinated, patient-centred care.
Hestia Health Duran i Reynals was represented by Dr José Manuel Núñez, Medical and Clinical Care Director, who took part in the panel on coordination between levels of care. Alongside representatives from GAPiC Delta and the hospital itself, the panel explored different perspectives on improving connections between primary, hospital and integrated health and social care.
In his contribution, Dr Núñez explained the intermediate care model used at the centre, highlighting its role as a bridge between the acute hospital and the community. He also emphasised the importance of effective coordination in responding promptly to complex clinical situations.
The conference provided a space for open dialogue between professionals from different disciplines, highlighting the importance of working together to provide care that better meets people’s real needs. At Hestia Health, this reinforces our commitment to comprehensive, approachable and coordinated care.
